As I've discovered yesterday ...... on your blackbox log a motor showing maxed out on your graph can also mean a desync occurred in that esc.
I'm using blheli_S dys xs30a escs and was having this issue.
Anyways I thought it was a good thing to share. So if you see a motor going to max it can also mean a desync.

first thing I would try is lower motor timing in ESC. But I have been hearing some desync issues with XS ESCs... what motors are you using?
Also try to update ESC firmware to the latest see if that helps.
